The typical way to address an envelope is *most* specific to least specific, as in
Name
Title
Mail-Stop or internal mail address
Department
Division
Company
Suite Number or Floor Number
Street Address
City, State, Zip
Country
This is obviously a really long address, and I doubt any company would routinely use all the items, but I put them all in, just in case. I'm not Miss Manners, and I haven't looked this up, but it makes sense and is the way that I've seen it done the most.
