I've used Microsoft's Natural Wireless Mouse 6000 and Comfort Keyboard 1045 for several years now. I don't think this Laser Mouse requires my thumb to be involved whatsoever. And I suspect the ergonomics have improved even further since mine was designed.
One problem I've heard is that it isn't very comfortable for people with very small hands. And some people claim that it is un-workable for the left-handed people among us. We're all a little bit different. Try one out . . . . whatever you want to buy, try one out first.
If you can't find a testing facility closer, you could come and try mine.
