I wanted to make a mortgage application for my website and I looked everywhere for a custom application feature. I found a couple of sites, but wufoo was the best. It has several of different custom form templates that you can use. Just thought it may be useful for someone.

Also check out www.FormAssembly.com - a bit more complex than wufoo, but much more in the way of options. I started off using wufoo for my forms, but switched to FormAssembly shortly thereafter.

If you use a template based site, it may not accept forms. But Wufoo is pretty cool.
