I ask and provide them the form but don't always get it back.

I see Concepts05 apparently does this with the purchase offer in MA. In NY we are required to do it at "the first substantive contact". therefore I do it as part of the listing process.

In OH, we do it with the offer as well, so it just gets sent in with that. We used to do it at the "first substantive contact" and then I sent them in with the other listing docs.

I really should be doing it with the listing agreement...I'll try to be more on top of it! (if its my own buyer, then I get it immediately) But I always, always get it by the offer.
